Dyspnea Plan of Care: Nursing
The Dyspnea Plan of Care is initiated and incorporated into the patient's plan of care when the OASIS item MO 490 is >= 2. Educational materials are left in the patient's home.
Effective Breathing:
- Assess effectiveness of patient's breathing and chart findings on appropriate portions of the SN note.
- Follow items I-IV on page 253 of Client Teaching Guides for Home Health Care by Aspen Publishers, Inc.
- Chart the plan of care steps on the SN Notes.
Energy Conservation:
- Follow the Energy Conservation plan as per OT and on page 257 of Client Teaching Guides for Home Health Care by Aspen Publishers, Inc.
Relaxation Technique:
- Follow the Relaxation Techniques on page 260 of Client Teaching Guides for Home Health Care by Aspen Publishers, Inc.
Muscle Strengthening:
- Reinforce the muscle strengthening plan developed by PT.
