Pain Management Program Plan of Care: Nursing
The Pain Management Program Plan of Care is initiated and incorporated into the patient's plan of care when the patient reports pain >= 3 on the 10 point scale on OASIS MO 420. Educational materials are left in the patient's home.
Medication Administration
- Teach purpose, dosage, route, frequency, side effects, and drug/herb interactions of medications prescribed by physician.
- Teach the difference between physical dependence on medication versus addiction.
Complementary Therapies and Methods of Pain Management
- Teach complementary methods of pain relief to be used in conjunctions with medications, as preferred by the patient and/or family/care giver and as appropriate for patient's condition:
- Guided Imagery
- Music
- Biofeedback
- Resting
- Teas, herbs, or other means of pain control that are culturally acceptable to patient
- Progressive Relaxation
- Massage
- Cognitive Distraction
- Positive Thinking
- Meditation
- Controlled Breathing
- Prayer
- Use of Heat and Cold
- Refer to PT for instruction in performing ADLs and OT for instruction in energy conservation and job simplification when client reports persistent pain >= 3.
- Teach good nutrition and restricted alcohol.
ADLs, Exercise and Mobilization
- Reinforce the pain management ADL and pain management exercises developed by PT as attached and as shown in plan in home packet.
- Walking and Moving About
- Exercising
- Changing Positions
- Supporting Painful Area
Energy Conservation and Job Simplification
- Reinforce the energy conservation and job simplification plan developed by OT as attached and as shown in the plan in the home packet.
